How often sugar reaches your teeth matters more than how much

The tooth surface responds to episodes rather than quantities, which is why a single dessert is less damaging than a packet of sweets eaten one at a time.

The mistake in thinking about totals

Ask most people how to protect their teeth from sugar and they will talk about eating less of it. That’s sensible for general health, and for teeth specifically it is the wrong variable. The tooth surface does not experience a daily total. It experiences a series of separate events, each with a beginning and an end.

Two people can consume the same amount of sugar in a day and face quite different risks. One has a dessert after dinner and nothing else. The other keeps a packet of candy on a desk and takes one every twenty minutes through a working afternoon. For the teeth, those are not remotely equivalent.

What happens during and after one exposure

Bacteria in the plaque film on your teeth metabolize fermentable carbohydrates and produce acid as a byproduct. The acidity at the tooth surface rises within minutes of sugar arriving, and while conditions stay acidic enough, minerals move out of the enamel — demineralization.

Then the recovery begins. Saliva dilutes and clears what remains, buffers the acid back toward neutral, and supplies calcium and phosphate that move back into the surface. Given time, the surface substantially repairs the loss, which is why not every acid exposure causes a cavity.

The whole cycle takes some time to complete — commonly described as tens of minutes, though it varies with the individual, with saliva flow, with what was eaten and with how much plaque is present. What matters is the shape of it: a drop, then a slow recovery.

A cavity is what happens when that cycle is repeated so often that recovery never finishes before the next drop arrives, and the losses accumulate over months.

Why grazing is the modern version of the problem

A century ago meals were more separated. Now a great many people eat and drink continuously in small amounts: coffee with sugar sipped over an hour, snacks at a desk, a sports drink taken slowly through a training session, a bottle of juice carried around all afternoon.

Each sip restarts the clock. A sweetened drink consumed over ninety minutes is not one exposure, it is a long series of them, and it can be considerably more damaging to the teeth than the same drink finished in five minutes. That’s a genuinely counterintuitive conclusion and it is one of the more useful things I can tell people.

It also explains why some patients with careful diets and diligent brushing still develop decay. The habit doing the damage is often not perceived as eating at all.

What follows practically, without turning it into a rulebook

The general principle is to keep sugar to mealtimes where you can, and to finish drinks rather than nursing them. Between meals, water is the choice that leaves the surface alone, and in a fluoridated area it brings a small additional benefit.

It is worth knowing that the same reasoning applies to acidic things that contain no sugar at all, because the acid arrives directly rather than being produced by bacteria. That is a different mechanism with a similar frequency logic, and it is covered elsewhere on this site.

None of this is a prohibition. I am not going to tell anyone never to eat dessert, and a diet framed entirely as denial tends not to survive. The point is to shift when things happen rather than to eliminate them, because timing is the variable with the leverage.

Where individual advice has to take over

Risk is not the same for everyone. Someone with a dry mouth from medication has less saliva to run the recovery phase, so the same diet is more dangerous. Someone with exposed root surfaces is dealing with a material that dissolves at a milder acidity than enamel. Someone with deep grooves in their molars, or with braces, has more places for plaque to sit undisturbed.

That’s why generalized dietary advice can only go so far, and why a dentist who has looked at your teeth and asked about your day can say something more useful than an article can. If you are getting cavities despite a routine you believe is good, a detailed look at the timing of what you eat and drink is often where the answer is hiding.

And if you are not sure what counts, ask. Patients are frequently surprised by what falls into the category, which is a subject in its own right.

Common questions

Is it better to eat sweets quickly or slowly?

For the teeth specifically, quickly and with a meal. Prolonging the exposure extends the period of acidity at the tooth surface, so the same quantity does more damage when it is spread out. That’s the opposite of most dietary advice, which is why it is worth stating plainly.

Does it help to rinse with water after something sweet?

It helps a little with clearance, and it is a reasonable habit, particularly when brushing is not practical. It does not substitute for the frequency principle, and after something acidic, rinsing with water is generally preferred to brushing immediately.

Do sugar substitutes solve this?

They remove the fermentable carbohydrate that bacteria use, which addresses one half of the problem. Many products sweetened without sugar are still acidic in themselves, and that acid affects the tooth surface directly, so the label alone does not settle it.
